Session details: The extract comes from a collection of video-recorded Advance Care Planning discussions between palliative patients, their health proxy and a physician. The objective is for the patient to make decisions about her/his future care plan, especially about treatments and interventions he/she would want or not want (eg. being resuscitated, going to intensive care), and to document them. Globally speaking, various interventions can prolong life (“interventional care”), but for a patient with advanced disease, these come at the cost of quality of life (such as being bedridden or hospitalized for a long time). Decisions made during Advance Care Planning apply if the patient would lose her/his capacity for decision-making, and would be enacted by the health proxy. The physician’s role is that of a “facilitator”: to assist the patient in making these decisions, to explain the interventions.
